New Patient Appointment – 
Office Visit – 
MyChart Video Visit – EST – Video visits for established patients
Telemed – MyChart – New – Video visits for new patients

When selecting Insurance Information please note the following if you select “No Insurance”:

Patient Pays – Select this option if the patient, themselves, is financially responsible for the visit. The patient may wish to separately schedule an appointment with our Eligibility department to find a path to making the appointment more affordable. Learn more here.

Someone Else Pays – Select this option if a parent or guardian will be financially responsible for the visit.
